It's not too difficult - make a new terrain type that looks like water but actually is like a floor. Separate that from the coast with a special node that checks for your "swimming" skill level and will only let you enter if you can swim. Then you can walk around and fight the monsters. Yes, BoE can do that with Increment Flag. You can choose how much you want to increment or decrement the flag. Falling Stars actually has a system exactly like this. For stealing and breaking into homes, your karma goes down, and nobody will give you quests or join your party. There will be no Avernum 4. In Blades of Exile, At the Gallows serves as an Exile IV, but it will most likely remain on the Exile side.
