Are you referring to the Fiesty Slap of Pain building?

If so, there really is nothing more to do there. I think Jeff just added it for a spot of fun and levelling up.

C_man_uk is wrong. After the crystal cave in the Madhouse there is a small cave which leads to a building. I believe this is the building that you are referring to. Anyway, to get in the building you have to pass the small little fence that shows up and gets in your way. To do that, you need to exit the crystal cave from the northernmost exit which is in reality a secret passage. Once you get into the building fight your way to the leader of the brigands, an eyebeast. The town Mertis is the town that gives you the quest to eliminate the Madhouse.

I am the way into the doleful city
I am the way into eternal grief,
I am the way to a foresaken race.
Justice it was that moved my great creator;
Divine omnipotence created me,
And highest wisdom joined with primal love.
Before me nothing but eternal things
Were made, and I shall last eternally
Abandon all hope, all you who enter.

How do you get into the Brigands Madhouse?

Enter from the east entrance, and walk left. You'll find a secret passage in the rock. Walk down and turn right to leave the passage. Then walk up and you'll be teleported in.
