When you fall through the trap door you'll end up in a little underground cell. Once in your cell, there is a secret passage to the west. Go through it. Fight your way through to a stairway up. You'll be back in the giant's fort proper. To escape, you'll need to turn the wheel near the trap that you fell through orignally. You can either hunt down and kill the Giant Chief for a key, or have a party with a high STR. Do not go out the same way you got in.
